Hi Product Hunt! I made 5S Audit & Action Tracker because a 5S walk only helps if the findings turn into actions that someone follows up on.
It's a Notion template, not an app: five connected databases (Areas, Checklists, Audits, Audit checks, Actions), six page templates, a dashboard with audit and action views, English setup and field guides with fictional worked examples, and three editable question sets to start from.
Each audit gets its own copy of the questions from your library, so later library edits don't rewrite past audits. Applicable checks are rated 0 to 4 with equal weighting; justified N/A answers are excluded and unanswered checks stay separate from zero scores.
Honest limits: you need a Notion account, desktop setup is recommended, owner names are plain text (assigning an action doesn't send a notification), and copying questions and reviewing evidence are steps you do yourself.
